What is social baseline theory, and what does it say about a trusted person's presence?
Social baseline theory holds that the human brain evolved to expect proximity to trusted others as a basic resource, in much the same way it expects oxygen or glucose. When that resource is present, the brain and body appear to spend less energy preparing for and recovering from threat. The word baseline in the theory's name points to this idea directly, that other people function as part of a person's default operating state, not as an occasional bonus layered on top of an otherwise self-sufficient system.
The theory comes out of the University of Virginia lab of psychologist James Coan, developed with colleagues including Lane Beckes and David Sbarra. Its central argument, laid out in papers including Social Baseline Theory: The Role of Social Proximity in Emotion and Economy of Action and a later update, Social Baseline Theory: The Social Regulation of Risk and Effort, is that people are not built to regulate stress alone. Coan and Sbarra describe the nervous system as operating on an economy of action. Facing a threat or a hard task by yourself costs more, metabolically and neurologically, than facing that same threat or task with a trusted, familiar person nearby. The brain appears to fold close relationships into its working model of the self, treating another person's presence as a kind of outsourced resource for managing risk and effort. Coan has argued that this reframes what it means to need people. Needing consistent access to a trusted other looks less like a personal weakness to be corrected and more like a baseline design feature of a highly social species.
This is a meaningfully different claim than saying company is comforting or improves mood. Social baseline theory is a claim about physical economy, that proximity to a trusted person changes how much work your brain and body have to do to handle a given stressor. That distinction, workload rather than mood, is what the rest of the evidence below tries to pin down with actual measurements of brain activity, stress hormones, and heart function.
What did the famous hand-holding study actually find?
In a 2006 study, sixteen married women were placed in an MRI scanner and shown threat cues while holding their husband's hand, a stranger's hand, or no hand at all. Holding their husband's hand measurably calmed activity in the brain regions that process threat and fear, more than holding a stranger's hand did, and far more than holding no hand at all.
The study, Lending a Hand: Social Regulation of the Neural Response to Threat, published by James Coan, Hillary Schaefer, and Richard Davidson in the journal Psychological Science, used a straightforward design. Women in happy marriages were shown a red X, signaling a one in five chance of a mild shock to the ankle, or a blue O, signaling safety, while holding their husband's hand, a stranger's hand, or nothing. Both spousal and stranger hand-holding calmed activity in regions tied to the body's basic threat response. Spousal hand-holding went further, and the effect was strongest in the couples who rated their marriages highest in quality. In those couples specifically, holding a spouse's hand reduced threat-related activity in three regions:
- The right anterior insula, involved in processing bodily distress
- The superior frontal gyrus, involved in regulating emotion
- The hypothalamus, a structure that helps trigger stress hormone release into the bloodstream
It is worth being upfront that the original sample was small, sixteen women, which is typical for fMRI research given its cost, and one reason the later replication described below mattered so much. The researchers wrote, "attachment figures act as emotion regulators in ways that strangers do not." A larger 2017 follow-up study by Coan and colleagues repeated the design with 110 people of different relationship types, married, cohabiting, dating, and close platonic friends. Relationship label mattered less than expected. What mattered most was how supported a person felt by the hand they were holding, a detail that matters for coaching relationships that are not romantic or familial.
Can another person's presence really lower stress hormones like cortisol?
Yes, in controlled studies, though the effect is more mixed by sex and context than a simple headline would suggest. Having a trusted supporter present before or during a stressful task has repeatedly reduced cortisol release compared to facing the same task alone or with a stranger, though not identically for everyone.
The clearest early evidence came from a 1995 study of sixty-six adults preparing to give a public speech. Men who had their girlfriend present for support showed significantly lower cortisol responses than men who prepared alone or with a stranger. Women in the same study did not show the same benefit, and some showed a slight increase in cortisol when supported by their boyfriend, even though they rated the support more positively than men did. That sex difference has never fully disappeared from the research, and it is a useful reminder that presence helping the body is a real, replicated finding, not a universal law that works identically for everyone in every study. A separate study of thirty-seven men found that social support from a close friend suppressed the cortisol response to a standardized stress test, and that support combined with the bonding hormone oxytocin produced the lowest stress hormone levels and least anxiety of any condition tested.
There is also a longer-term picture. A 2023 follow-up to Coan's hand-holding research measured brain activity in the hypothalamus and related regions while participants held a supportive hand, then tracked their physical health years later. The people whose brains calmed the most in the moment were the same people for whom feeling socially supported translated, years later, into better heart rate variability and lower levels of a key inflammation marker called C-reactive protein. How much your brain relaxes into another person's support right now appears to say something real about your longer-term physiology, not just how you feel that day, which matters in early recovery specifically, a period already marked by significant physiological strain.
Is co-regulation, one nervous system steadying another, real?
Yes, in the sense that a caregiver's presence and behavior can measurably shape another person's stress biology, an effect first documented in early life and now studied between adults as well. The clearest foundational evidence comes from developmental researcher Myron Hofer, whose work beginning in the 1980s found that specific components of a mother's presence, her warmth, her touch, the rhythm of her feeding, each regulated a distinct physiological system in her infant, from heart rate to growth hormone to sleep cycles, independent of anything the infant could consciously understand.
Hofer called these findings hidden regulators, because the regulatory effect is only obvious once it is removed. Coan's own hand-holding research explicitly builds on this idea, describing attachment relationships as regulators that stay hidden until a person is under threat or the relationship is taken away. Developmentally, this line of research matters beyond the moment of contact itself, since consistent co-regulation early on is understood to help build a person's later capacity to regulate their own stress response, not only to soothe them in the moment.
Whether this extends cleanly from infants to adults is a fair question, and the honest answer is that the adult evidence is real but more limited. Psychologists David Sbarra and Cindy Hazan proposed a formal model of adult co-regulation, arguing that romantic partners' physiologies become intertwined over time, and that losing that partner produces measurable biological dysregulation, not only sadness. Separately, researchers have measured actual heart rate synchrony between romantic partners during structured conversation, finding real, moment to moment coupling between two people's cardiovascular systems. A 2015 review of this broader physiological linkage literature found the effect is genuine but context dependent. Syncing with a partner's body can reflect healthy co-regulation during a hard moment, or two people spiraling together during conflict. Presence is not automatically calming. It matters who the person is and what is actually happening between you. None of this is about whether people in general benefit from having friends. It is about the narrower, more mechanical question of whether one nervous system can measurably nudge another's, and on that specific question the evidence, while still developing, is real.
What does polyvagal theory add, and where is the science still contested?
Polyvagal theory, developed by Stephen Porges starting in 1994, proposes that the nervous system moves between calm social engagement, fight or flight, and shutdown states, guided by a subconscious safety-detection process Porges later termed neuroception. The core idea, that the nervous system has distinct calm and activated states shaped by perceived safety, is broadly useful, but several of the theory's specific claims about anatomy and evolution are actively disputed by other scientists, including in a formal 2026 critique.
In Porges's original 1995 paper, the three proposed states are, in plain terms:

That exchange is unresolved, and the debate remains active in the scientific literature. What is not seriously in dispute is the underlying phenomenon the theory is trying to explain, that the autonomic nervous system has real, measurable calm and activated states, that heart rate variability is a legitimate window into stress and recovery, and that safety cues, including another person's calm presence, appear to shift people toward a more regulated state. Does presence have to be physical, in the room, to matter?
Physical, in-person presence is not strictly required for someone to feel supported, but it is where the strongest and most consistent physiological evidence currently exists. The hand-holding research, the cortisol studies, and the heart rate synchrony findings described above all involved two bodies in the same room, often in physical contact, not a voice on a phone or a message on a screen.
That does not mean phone or video support is worthless. Feeling supported in general is associated with better stress patterns even outside a lab, and people plainly benefit from a supportive voice when that is what is available. But touch and physical proximity appear to add something specific. Hofer's original studies found that maternal warmth and tactile contact regulated infant physiology directly, through no mechanism resembling a phone call. Coan's hand-holding studies used actual touch, not eye contact or conversation, as the tool of regulation. The honest summary of the evidence is that trusted presence works on a gradient. A familiar voice helps. A familiar face on a screen likely helps more. Physical closeness with someone you trust appears to do the most, at least based on where researchers have pointed their instruments so far. The original hand-holding researchers proposed touch itself as a plausible trigger, noting that physical contact is associated with oxytocin release from the hypothalamus, though that specific chain of events was proposed as a likely mechanism rather than directly measured in their study. That is a meaningful design question for any model of support, virtual, phone based, or in person, and it is one this research area is still actively working through.
